Lipid Lowring Drugs
The role of hyperlipoproteinaemia as a risk factor in coronary heart disease and cerebrovascular disease is fairly well established. The drugs available for the treatment of hyperlipidaemia vary markedly in their chemical structure, their mechanisms, and their clinical response.
The available drugs are:
I. Predominantly effective in hypertriglyceridaemias:
Fibrin Acid Derivatives: Clofibrate, gemfibrozil, fenofibrate, bezafibrate.
II. Predominantly effective in hypercholesterolemia
Bile Acid-Binding Resins: Cholestyramine, colestipol, HMG-CoA Reductase Inhibitors: Lovastatin, atorvastatin, fluvastatin, pravastatin, simvastatin.
Ill. Miscellaneous Drugs: Nicotinic acid probucol, dextrothyroxine, safflower oil.
Clofibrate
Clofibrate is used in patients with hypertriglyceridaemias or hypercholesterolemia to reduce the respective blood levels. It has shown favorable results in the long-term treatment of hyperlipidaemia.
Gemfibrozil
Gemfibrozil is a structural analogue of Clofibrate and shares many of its pharmacologic and toxicologic properties. It lower elevated serum triglycerides, mainly the VLDL fraction, and to a lesser extent the LDL fraction Fenofibrate: Fenofibrate is a relatively newer agent. It has the same indications as gemfibrozil, and is used widely in Europe It is yet to be available in other countries for general use.
Bezafibrate
Bezafibrate induces appreciable reduction in plasma triglycerides, VLDL, and LDL cholesterol. HDL levels are raised.
Colestipol
Cholestyramine is the chloride salt of a quaternary ammonium anion-exchange resin, which in the intestinal lumen binds bile acids, exchanged for chloride. This insoluble resin-complex is not absorbed, and thus it promotes the fecal excretion of bile acids which would have been otherwise largely reabsorbed from the intestines. Such a mechanism may lower serum cholesterol.
Lovastatin
Lovastatin is an inactive lactones. In the body it is hydrolyzed to a beta-hydroxyl acid form, which is a potent inhibitor of HMG-CoA Reductase, an enzyme that catalyzes the conversion of HMG-CoA to Mevalonate, a rate-limiting step in the biosynthesis of cholesterol. Both normal and elevated LDL cholesterol levels are lowered.
Simvastatin
Simvastatin, a HMG-CoA reductase inhibitor, is a prodrug which is bioactivated in the liver to form the active beta-hydroxyacid derivative.
Nicotinic Acid
Nicotinic acid when administered in large doses of 1.5 to 8g daily, lowers plasma triglyceride and cholesterol levels. Probably it acts by inhibiting the release of free fatty acids from adipose tissue, and inhibits cholesterol synthesis in the liver.
Probucol
Probucol decreases elevated serum cholesterol levels by reducing low-density lipoproteins (LDL) concentrations but does not reduce serum triglyceride levels appreciably.)
Dextrothyroxine
The dextro-isomer of thyroxin increases the rate of oxidation or hydroxylation of cholesterol to bile acids in the liver, and promotes the biliary excretion of cholesterol.
Diuretics are commonly defined as drugs that increase the amount of urine produced by the kidneys. A precise definition is that diuretics are agents which augment the renal excretion of sodium and either chloride or bicarbonate primarily, and water excretion secondarily. Clinically diuretics are the mainstay of treatment of hypertension and edematous states like congestive heart failure, cirrhosis with ascites, nephritic edema, and edema or pregnancy.
